Submit a Claim - Claim Instructions

To qualify for a payment, you must submit a timely and valid Claim Form with the required supporting documentation. The Claim Form must be submitted by email such that it is received no later than December 28, 2020, or, if mailed, be postmarked no later than December 28, 2020.

You may submit the Proof of Claim Form:
